Vending Machine Refund Request

Complete this form to request a refund of payment lost to a malfunctioning vending machine. To report multiple incidents, complete and submit this form for each incident.

Please note: If you do not see the applicable building listed in the "machine location" field below, your refund request cannot be processed via this form; please follow any refund processing instructions listed on the machine in question or contact building management directly for resolution.

Refund requests must be submitted within five (5) business days of incidence. Refund requests submitted more than five business days after an incidence of malfunction cannot be reimbursed.
